Good medical practice requires the oversight of properly trained and credentialed physicians to supervise and evaluate the work of registered professional nurses who are limited in their scope of practice by their license level.  Having physician oversight would also allow for increasing the types of services provided.  The consulting physician would be hired to work 2 hours per week.
